The Program Implementation Specialist will be placed primarily at Community Pediatric Programs (CPP), the flagship project within the Children’s Health Fund (CHF) network. CPP often works together with the CHF National Office Department of Medical Affairs to develop new projects, field test initiatives, develop best practice models, or modify them for the health center or mobile medical unit model. The Program Implementation Specialist will share time between Children’s Health Fund, located in Manhattan, and CPP, located in the South Bronx. At CHF, the Program Implementation Specialist will be involved in the design and evaluation of various projects throughout the CHF network. At CPP, the Program Implementation Specialist will be responsible for supporting the Resiliency Initiative. The mission of the Resiliency Initiative is to identify and mitigate toxic stress in pregnant women and parenting families through universal screening for parental stress and support in prenatal and pediatric care, the provision of group prenatal and pediatric care, intensive counseling for families in need, linkages to community resources, and maternal-infant dyadic therapy. This work will require the Program Implementation Specialist to directly assist with organizing and running group care, as well as helping to institutionalize social determinant of health screening in primary care visits. Additionally, the Specialist may be involved at various levels of data collection, patient, and community outreach efforts, and project management. This work will involve working closely with patients, as well as with clinical teams of physicians, nurses, nutritionist, and mental health specialists as well as the staff of the Starr Center, CPP’s program planning and evaluation team.

Work with Community Pediatric Programs to support program design, implementation, management, and evaluation of selected projects
